The current role of elective neck dissection in management of early stage oral tongue cancer; a retrospective study

Abstract: Background: Occult neck metastasis rate is too high in tongue cancer. Objective: To verify the current role of elective neck dissection (END) in management of early stage oral tongue cancer to minimize disease failure and improve survival. Study design: This retrospective study included 88 patients with stage I and II tongue cancer. The collected data were analyzed for detection of disease free survival (DFS) and recurrence rate. Results: Most lesions had been dealt with by surgery, either by wide local excision (22%) or hemiglossectomy (78%). The treatment of the neck was either by neck dissection (85.2%) or “wait and see” policy (14.8%). The rate of local and nodal recurrence was 7.9% and 20.4% respectively. Analysis of the association between DFS and different factors revealed significance associations with adoption of adjuvant therapy and the status of dissected lymph nodes. Conclusion: Controversy as regards neck management was still present.

The Relationship between Body Mass Index and Thyroid Cancer Pathology Characteristics and Outcomes

Abstract: Background: Differentiated thyroid cancer (DTC), especially of the papillary carcinoma (PTC) type accounts for the most common endocrine cancer worldwide with an escalating trend for several decades. This might be partially due to improvement in modern diagnostic imaging and biopsies along with the shift in genetic and environmental factors like ionizing radiation, iodine consumption, and lifestyle modifications. Obesity prevails as one such crucial factor that is long known to be controversially involved in the emergence of thyroid cancer with no clear understanding of the precise mechanism. Our study wanted to determine the relationship between body mass index (BMI) and clinicopathological characteristics of differentiated thyroid cancer (DTC). Methods: Data were retrospectively traced from the medical records of 124 patients in total, (19.4% male and 80.6% female) DTC patients who were operated on for the last 16 years in this study. Their TSH levels, weights and heights were recorded before operation. The BMI was calculated and correlated with the histopathological findings. One year postoperatively the DTC outcome was determined based on 2015 ATA DTC guidelines. Result: The results thereby demonstrated no positive associations between BMI and diagnostic stage of the tumour (T, N, or M stage), vascular invasion, capsule invasion and Extra thyroid extension (ETE) concerning BMI quartiles. Upon univariate and multivariate analyses, an unexpected inverse association was identified between BMI and nodal metastasis and tumour invasion. This is suggestive of the statement that less aggressive tumour features may be associated with obesity. Conclusion: The results lay out the relationship that aggressive features of thyroid cancer may not be indicated by obesity. However further analysis is required to critically estimate the underlying mechanism between the risk of thyroid cancer and obesity.

The effect of training with addition weights, on some Biokinematic Variables for half rotate stalder on chin device for beginners.

Abstract: The Aim of the search is to prepare exercises with addition weights to improve kinematic variables and to identity the effect of training by weights on these variables for the half turn stalder skill on chin device for juniors . The two hypothesis were exist positive effect on some kinematic variables of the benefit of post-test. The researcher used experimental design with video, the subjects were (6) gymnastic players chosen internationally from Baghdad district. The chapter four included the display analysis and discussion. The researcher concluded that the exercises with addition weights contributed to improve some kinematic variables  for the half turn stalder skill on chin device for beginner. Finally the researcher recommended doing exercises with addition weights on other gymnastic skills and kinematic variables values, when training half turn of stalder on chin device for beginners.
